Help Toronto Central LHIN Improve Health Services For You!

As you may know, CCAC (Community Care Access Centres) was transformed into LHINs aka Local Health Integrated Networks. For those of you based in Toronto with the exception of North York which is served by Central LHIN and not Toronto Central LHIN (confusing right?), they are seeking feedback to understand what they can do better serve you and your family.

The Toronto Central Local Health Integration Network (LHIN) is currently developing a new strategic plan for 2018 to 2022. Your feedback, as residents of Toronto receiving health care services, is really important for the Toronto Central LHIN as it plays a key role in working with local health service providers and community members to determine the health system priorities for the region. Please find below a link to a short 5-minute survey that seeks to understand what is most important for your health and health care in Toronto. Your participation in this short survey will go a long way in helping the Toronto Central LHIN understand the actions needed to make the biggest impact on your health.
